3. What is the reason behind it?
The direct cause of many safety accidents is human illegal operation. For example, on February 28, 2012, a major explosion occurred in a workshop producing guanidine nitrate at a chemical plant in Shijiazhuang, Hebei Province, resulting in 25 deaths, 4 missing and 46 injured.
The direct cause of the accident was the unauthorized implementation of relevant changes without in-depth safety risk assessment, which changed the production raw materials, reformed the thermal oil system, and raised the maximum controlled temperature of thermal oil from 210℃ to 255℃, causing the reaction product guanidine nitrate and incomplete ammonium nitrate in the reactor to be heated locally, rapidly decomposed and exploded. This triggered the explosion of guanidine nitrate and ammonium nitrate stored nearby.
